Welcome, at Shalom Pediatric Clinic, we aim to offer high quality Pediatric Medical Services to our community in general, but with personalized attention to each child’s medical needs. We believe your child deserves the best medical attention and treatment in a welcoming, caring and warm environment. Our dedicated staff strives to provide that, and much more, to your child!

We gladly provide the following services to our valued patients:
Full Physical Examination for Camps, Sports and School (K-12).
Immunizations (shots) and flu shots
Consultation for overweight, eating disorders, skin disorders, etc.
Same-day treatment for all sick children and adolescents
Management of asthma

Diagnosis of ADHD

Medical Isurance
We accept most government and third-party insurances. Please, call our office for more information.
Insurance Information and Bill Payments
Please, always cross-check with your insurance for updates, or other changes, concerning your benefits. Notify us of such changes during your visit.
You are required to present your insurance cards, and other relevant information, at each visit to enable us submit your claims timely.
If the insurance information are wrong or denies payment, you are responsible for payment of the bill.
Your insurance determines your eligibility and benefits. However, you pay any part of the services not covered by your insurance benefits.
Financial Policy for Shalom Pediatric Clinic, PLLC
We will work with you to ensure that your healthcare cost is very minimal. However, we require your co-operation to make this a possibility
NO SHOWS: If you plan to re-schedule your appointment, call us 24 hours before the appointment is due. Other patients may need the slots. No shows will be billed $15.
MEDICAL RECORDS FROM SPC: We require, at least, 48 hours notice to process your medical records. A minimum processing fee of $25 is required for one patient; $35 for a family. Patients must complete, “Authorization to Release Medical Record” form before the medical records are released or transferred. The form is available at our office.
NO MEDICAL INSURANCE: We will offer reasonable discounts and free estimates of your bill with you. But all bills will be paid before services can be provided.
OTHER PAYMENTS: Our insurance obligations require us to collect all co-pays, coinsurances, deductible and other balances before services can be provided.
FORMS: To fill-out and process all forms, we charge a processing fee of $5 for each form.
Patients who do not comply with this policy will be liable for any debt incurred. If you have any billing questions, our billing question and insurance specialist will be happy to assist you further. We will make possible efforts to contact you to pay your bills. Unpaid bills will have 20% monthly interest added to it until paid. In addition, unpaid bills are sent to our collection agents.
AGREEMENT: This whole agreement is between Shalom Pediatric Clinic, Pllc, “we” and the Patient/Guardian/Debtor, “you.” By signing below, I understand that I am responsible for fulfilling all the financial policies as outlined above, on behalf of the Patient(s).

Frequently Asked Questions (FAQ's)
Q. What are the office hours?
A. Monday to Thursday: 9:00 am – 5:30 pm
Friday: 9:00 am - 1: 30 pm
Saturday: By appointment (requires 48hrs notice)
Sunday: Closed
Q. Is my child with the acceptable age group?
A. We serve patients from newborn to 21 years of age.
Q. Are the doctors American Board Certified?
A. Yes, all the doctors are American Board Certified Pediatricians (FAAP).
Q. Do I need my insurance card for clinic visits:
A. Yes, you will need your insurance card any time you visit the clinic for services.
Q. Can I have access to my child’s immunization records?
A. Yes, all immunizations/vaccines administered in our clinic are entered into the NC Immunization Records (NCIR) electronically. This also enables other Healthcare providers, who have assess to NCIR, to have update information of your child’s immunization status.
Q. My child needs shots, what do I need to do?
A. Get a copy of your child’s immunization records ready during your child’s scheduled appointment. However, if your child is not up-to-date with his/her shots, the doctor will plan a “catch-up” schedule.
Q. I am a new patient, what do I need to do?
A. We have made efforts to ensure that your visits to our clinic is always hassle-free and timely. You will need a copy of your child’s insurance, driver’s license or any official ID of the parent/guardian, copy of your child’s immunization records. New patients will be required to do some paper work – usually a few pages.
Q. Has your child accidentally ingested a poison?
A. Immediately call Poison Control Center: 1-800-222-1222.
